You have a client that is ready to sign up for the Kai-Zen program. Congratulations! We appreciate your business. Please start with Step 1 of the 3 step process below in order to get started.

All forms must have a physical signatures (wet signatures, with pen and ink) from the client. Electronic signatures WILL NOT be accepted.

Submit all of the following forms and application to: 

lifeinsurance@send.allianzlife.com and CC info@kaizenplan.com.

Please download and fill out the following forms. After submission, Allianz will order the medical exam.

1. Download & Complete the Kai-Zen  Cover Form

2. Download & Complete all four parts of the Allianz Nevada Life Insurance Application Form (ICC18-LIFE)

3. Allianz  Kai-Zen Premium Financing Disclosure

1. Producer’s Report Form #NB5089-01 -Make sure NIW/Grace Barnard is listed for 30% split –agent code is 673000101(Example Form and How To Complete)

2. Supplement to the Life Insurance Application -Personal Financial Statement Form #NB2012P (Example Form and How To Complete)

3. Allianz Premium Financing Standards Addendum aka Agent Addendum-Needed if agent has not done a financed case with Allianz in the past (Example Form and How To Complete)

5. Form #NBAL0013-NV – Replacement Packet (Example Form and How To Complete)

Once all forms are completed, signed and dated you will submit DIRECTLY to lifeinsurance@send.allianzlife.com & CC info@kaizenplan.com.

After the above documents have been submitted, Allianz will order the paramed exam. Contact Allianz for all medical & financial underwriting inquiries and questions.

E-APP Submission Has Different Requirements

If you are applying using the E-APP you MUST follow the E-APP process and order your own exam, etc. The Phone Interview will be required when doing the E-APP.

Proceed to Step 2 enrollment process once you have received a medical offer from Allianz.

Attention: Agents MUST be licensed in state of Nevada PRIOR to taking an application for all Allianz Kai-Zen cases!

When completing the application, please follow the instructions in the Allianz Enrollment Tutorial (above), it contains specific details that must be followed to avoid delays in processing.

Using this new application will eliminate the phone interview – Allianz will order the paramed exam.

Once you have the offer, forward the offer letter to illustrations@niwcorp.com to obtain final illustrations (Allianz Final illustration and the Kai-Zen Final Illustration). The email should include:

Kai-Zen illustrations will be provided to you within 48 hours of receipt of above requirements.

1. Color Copy/Photograph of Insured Driver’s License (address on DL must match address on application or Trustee will need a copy of utility bill ( water, gas & electric ) to confirm address on application. Or a notarized letter.

All the above forms and a copy of the client’s premium check should be scanned to info@kaizenplan.com.

Case documents are NOT reviewed for possible errors until premium check has been received.

*****Once the full (Step 3) paperwork including the premium check has been received, please allow 5-7 business days for processing. To avoid last minute follow up and to guarantee your client will participate in the current tranche, client’s premium check should arrive 1 week prior to tranche deadline date. *****

Polices are not in force until the bank has funded. Agents will receive an email alert the day the bank funds along with the Life Carrier’s ” delivery ” instructions. Then approximately within 3 weeks, clients will receive by email a ” Welcome Packet ” which will include all paperwork previously signed by the client along with the Trustee’s signed documents.
